Lost a Card? No Problem!

There is a $5 replacement fee. This gets you a replacement card - not a photocopy.

Just follow these easy steps:

1. Contact Don at (317) 965-8362.
2. Or……… send a self addressed stamped envelope with a class name and location and/or date if possible with your request along with your PRINTED NAME as you want it on your card.
3. Please send a signed check along with the request. Make checks payable to: CPR Institute of Indiana

Any returned or cancelled checks subject to a $35 returned check fee.
